Healing is not only achieved externally but is also required internally in order to ensure growth. Breathing life and bringing meaning to a persons life is where Occupational therapy comes in. “Occupational Therapy.. mm what on earth is that?” is a question I have heard more times then I can count. It is extremely difficult to answer that question as words do not express the amount of work dedicated OT's put towards the holistic healing of their clients. Taking into account our clients context and the environment they are returning to makes our profession meaningful. Occupational therapy gives the person a sense of autonomy, confidence and meaning in their life. Imagine a life without feelings of self-worth. In order to elicit these internal feelings we bring quality of life in an individual by driving them as close as possible to a life they want to live. It is important that Occupational therapy services are offered in hospitals, clinics, schools and communities.

 According to the World Health Organization “Primary Health Care (PHC) is essential health care based on practical, scientifically sound and socially acceptable methods and technology, made universally accessible to individuals and families in the community through their full participation and at a cost that the community and the country can afford to maintain at every stage of their development in the spirit of self-reliance and self-determination”. For paediatric clients the role of an OT is early detection of disability (Naidoo, Van Wyk & Joubert, 2016). Within the community of Mariannridge this role is fulfilled through community projects such as Meet the Milestones which educates parents in the clinic of developmental delays and the warning signs they must be aware of. This project also includes screening of children. Children with global developmental delays are seen for OT intervention while others are provided with client centered home programmes which are implemented by parents or caregivers to stimulate normal development. This project is efficient in health promotion and prevention as it allows for early detection of developmental delays and informs parents about the normal development children.

The OT role in school environments is to provide services for learners with learning disabilities, to work with adolescents and teenagers to promote well being by assisting them to develop life skills and discourage negative and self-harming behaviors such as unprotected sex, alcohol and drug use (Naidoo, Van Wyk & Joubert, 2016). In the community of Mariannridge this role is being fulfilled as OT 4 students conduct primary school interventions which focus on assessing and providing early intervention for grade 1 learners who experience learning difficulties. Projects such as the Adolescents and Youth Friendly clinic promote health among high school learning. Within this project topics on sexual health, substance use, peer pressure and overall health of a student are explored. Through these projects we are able to lessen the number of struggling children who are lost in the educational system as they are receiving the necessary help. With the Adolescents and Youth Friendly clinic we are able to decrease self-harming behaviors among the youth within this community.

 The role of OT in PHC also includes the health of families and the community in which one in working in (Naidoo, Van Wyk & Joubert, 2016). OT students provide services to families through home visits. These home visits provide essential rehabilitation services to individuals who required OT intervention. Many people within the community of Mariannridge are unable to access rehabilitation services due to physical constraints resulting from their disability or financial constraints due to their poor socio-economic status. OT students provide services to the community through community empowerment projects such as the Women's Empowerment Project and the Gogo project. The Women's Empowerment Project empowers women in the community by promoting skill development and providing income opportunities. The Gogo project provides a sense of belonging and unity among elderly women within the community.

 However, some OT's lack of clarity on the role of OT in health promotion and prevention (Naidoo, Van Wyk & Joubert, 2016). This is the reason why OT's remain hospital focussed and transfer a hospital approach to a PHC clinic thus leaving out health prevention and promotion as part of their services (Naidoo, Van Wyk & Joubert, 2016). As OT's we must ensure that our interventions take into consideration the context we are working in, the needs and overarching health issues of the community to ensure intervention is relevant for our clients. As OT's various forms of media can be utilised to when promoting health, preventing health and during our rehabilitative services.

In occupational therapy the use of media in our health interventions can lead to greater accessibility of vital information to people and promotion of our profession as it remains under recognized.

 Examples of successful use of media in health is the listeriosis outbreak which occurred earlier in the year. Through the use of media platforms such as newspapers, TV, radio, social media platforms including Facebook, Instagram and Twitter the public was informed of this outbreak. Without the use of media a greater population of people would have been affected by the outbreak. Other uses of media for health promotion and prevention includes Its Starts With Me Summer 2018 campaign which launched in June 2018. This campaign promoted awareness that effective HIV treatment leads to a full quality of life. This campaign challenged stigma associated with HIV by sharing stories of HIV people who are living fulfilled life's.

 As occupational therapists it is important to appeal to the greater masses. This is only achievable if we adjust to the trends which are current in the times we are living in.

Respect A Mom's Choice To Bottle Feed Her Baby, Says Royal College Of Midwives

The debate of bottle versus breast is rarely without controversy.

While the world’s top health experts recommend exclusive breastfeeding until a child is six months old with continued breastfeeding to age two and beyond, campaigns such as “Fed Is Best” point out that women often feel immense pressure to only breastfeed their newborns, “even when they do not have enough milk to do so.”

And as breastfeeding moms are sometimes applauded for nursing in public (although many are also shamed for doing the same, because there’s no winning), those who don’t breastfeed say they feel “judged at every turn.” Recent research suggests 70 per cent of moms feel negatively judged for bottle-feeding their babies, according to the Mirror.

Well, The Royal College of Midwives in the U.K. is saying ENOUGH. In a new position statement, the college emphasized that the decision to bottle feed is a woman’s choice that must be respected.

“Evidence clearly shows that breastfeeding in line with WHO guidance brings optimum benefits for the health of both mother and baby. However the reality is that often some women for a variety of reasons struggle to start or sustain breastfeeding,” chief executive Gill Walton said in a news release today.

“If, after being given appropriate information, advice and support on breastfeeding, a woman chooses not to do so, or to give formula as well as breastfeeding, her choice must be respected.”

Parents who want to feed their babies with formula (whether it’s exclusively or in addition to breastfeeding) should be given balanced and relevant information “to enable them to do so safely and with support to encourage good bonding,” the statement said.

“We recognize that some women cannot or do not wish to breastfeed and rely on formula milk. They must be given all the advice and support they need on safe preparation of bottles and responsive feeding to develop a close and loving bond with their baby,” Walton said.

The new statement is significant because medical communities tend to advocate for breastfeeding’s health benefits.

In Canada, exclusive breastfeeding for the first six months is recommended by Health Canada, the Canadian Paediatric Society, Dietitians of Canada and the Public Health Agency of Canada, according to a 2015 report by Statistics Canada.

In a 2008 joint statement, The Canadian Association of Midwives and the Canadian Nurses Association advocated for breastfeeding while still noting that the decision is a woman’s choice.

“While supporting a woman’s right to decide the method of feeding her child, registered midwives and registered nurses in Canada are strong advocates of breastfeeding and will work with their clients, other health-care professionals and public policy-makers in all sectors as necessary to encourage successful breastfeeding,” the statement read.

Why women stop breastfeeding

The majority, or 89 per cent, of mothers in Canada breastfed their baby in 2011-2012, Statistics Canada notes, but far fewer, 26 per cent, breastfed exclusively for the full six months.

“The most common reasons cited for stopping breastfeeding before six months were ‘not enough breast milk’ and ‘difficulty with breastfeeding technique,'” the agency said.

The Canadian Paediatric Society notes that “if breastfeeding is not an option,” parents can use a store-bought iron-fortified infant formula for the first nine to 12 months.

In 2017, grieving B.C. widower Kim Chen made an emotional plea for new mothers not to feel guilty if they’re unable to breastfeed, and urged hospitals not to put so much pressure on women.

His wife, Florence Leung, took her own life after suffering from postpartum depression.

“Do not EVER feel bad or guilty about not being able to ‘exclusively breastfeed’, even though you may feel the pressure to do so based on posters in maternity wards, brochures in prenatal classes, and teachings at breastfeeding classes,” Chen wrote in a Facebook post.

“While agreeing to the benefits of breast milk, there NEEDS to be an understanding that it is OK to supplement with formula, and that formula is a completely viable option.”
